MyHealth App Privacy Policy

Axios International Consultants Ltd, its subsidiaries, affiliates, and partners (“us”, “we”, “our”, or “the Company”) operates the Patient App (the “Services” or “Products”).

This policy describes the information we process to support the Services, Products and features offered by the Company.

To provide our Products, we must process information about you. The types of information we collect depend on the type of user you are, and how you use our Product. We also use your data to provide and improve the Services. By using the Services, you agree to the collection and use of information in accordance with this policy. Unless otherwise defined in this Privacy Policy, terms used in this Privacy Policy have the same meanings as in our Terms and Conditions. Please read this Privacy Policy carefully.

AGREEING WITH THE TERMS OF THIS POLICY IS MANDATORY FOR USERS TO ACCESS THE APPLICATION.

OUR ONLINE ECOSYSTEM

Our products are interrelated and self-contained within the Company domain. The mobile applications aforementioned and subjects of this Policy are feeding information into the Patient Management System, hosted on the Company domain and for which our Terms and Conditions apply.

INFORMATION COLLECTION AND USE

We collect, directly or through participation in any of the Application’s activities, several different types of information for various purposes to provide and improve our Service to you. The information we collect depends on the content and material you use.

Types of Data Collected

Personal Data

While using our Services, we may ask you to provide us with certain personally identifiable information that can be used to contact or identify you (“Personal Data”). Personal, identifiable information may include, but is not limited to:

  • Cookies
  • Usage Data

Through your utilization of our Services, you also provide us with personal data that we store on our Products in order for you to access them from a centralized, secure platform. This may include:

  • First name and last name
  • Email address
  • Postal address
  • Phone and Fax number

Device Information

As described below, we collect information from and about the computers, phones, connected TVs and other web-connected devices you use that integrate with our Products, and we combine this information across different devices you use. For example, we use information collected about your use of our Products on your phone to better personalize the content or features you see when you use our Products on another device, such as your laptop or tablet.

Information we obtain from these devices includes:

  • Device attributes: information such as the operating system, hardware and software versions, battery level, signal strength, available storage space, browser type, app and file names and types, and plugins.
  • Device operations: information about operations and behaviors performed on the device, such as whether a window is foregrounded or backgrounded, or mouse movements (which can help distinguish humans from bots).
  • Identifiers: unique identifiers, device IDs, and other identifiers
  • Data from device settings: information you allow us to receive through device settings you turn on, such as access to your GPS location, camera or photos.
  • Network and connections: information such as the name of your mobile operator or ISP, language, time zone, mobile phone number, IP address, connection speed and, in some cases, information about other devices that are nearby or on your network, so we can do things like help you stream a video from your phone to your TV.
  • Cookie data: data from cookies stored on your device, including cookie IDs and settings. Learn more about how we use cookies in the section below.

The device ID and device ID advertising are used to send you personalized push notifications, respectively to inform you on the Services itself and to keep you informed of changes and updates of the Products. We do not use the device ID and device ID advertising to send you marketing or promotional content.

The data collected on your camera and photos is used in the process of you uploading relevant documents to your activity through our Services.

Tracking & Cookies Data

We use cookies and similar tracking technologies to track the activity on our Services and hold certain information.

Cookies are files with small amount of data which may include an anonymous unique identifier. Cookies are sent to your browser from a website and stored on your device. Tracking technologies also used are beacons, tags, and scripts to collect and track information and to improve and analyze our Services.

You can instruct your browser to refuse all cookies or to indicate when a cookie is being sent. However, if you do not accept cookies, you may not be able to use some features of our Services.

  • Session Cookies: we use Session Cookies to operate our Services.
  • Preference Cookies: we use Preference Cookies to remember your preferences and various settings.

Our Services use the collected data for various purposes, especially to:

  • provide and maintain the Services
  • notify you about changes to our Services
  • allow you to participate in interactive features of our Services when you choose to do so

  • provide analysis or valuable information so that we can improve the Services
  • monitor the usage of the Services
  • detect, prevent and address technical issues

Axios is a Data Processor. In simple terms this means Axios processes patient data on behalf of our Clients. In some instances, Axios and its Clients act as joint Data Controllers where it is agreed for which and how personal data is processed.

We share information globally, both internally within the various Company offices across the world, and externally with our partners under this policy. Information is processed by the Company in Ireland. It may be transferred or transmitted to, or stored and processed in other countries outside of where you live for the purposes as described in this policy. These data transfers are necessary to provide the services set forth in our Terms and Conditions, and to globally operate and provide our Service to you. We utilize standard contractual clauses approved by the European Commission and rely on the European Commission’s adequacy decisions about certain countries, as applicable, for data transfers from the EEA to the United States and other countries.

Your consent to this Privacy Policy followed by your submission of such information represents your agreement to that transfer.

The Services will take all steps reasonably necessary to ensure that your data is treated securely and in accordance with this Privacy Policy and no transfer of your Personal Data will take place to an organization or a country unless there are adequate controls in place including the security of your data and other personal information.

EXERCISING YOUR RIGHTS PROVIDED UNDER THE GDPR

Under the General Data Protection Regulation, you have the right to access, rectify, port and erase your data. Learn more about these rights and find out how you can exercise your rights in the App Settings. You also have the right to object to and restrict certain processing of your data.

Accessing or Rectifying Your Personal Data, Deletion, Deactivation, Object, Restrict, or Withdraw Consent and Portability: upon request, we can assist in updating, deactivating or deleting your personal data. See the Contact Us section below for means to reach out.

There may be limitations on our ability to comply with your request.

DISCLOSURE OF DATA

Legal Requirements

The Services may disclose your Personal Data in the good-faith belief that such action is necessary to:

  • comply with a legal obligation
  • protect and defend the rights or property of the Service or the Company
  • prevent or investigate possible wrongdoing in connection with the Service
  • protect the personal safety of users of the Service or the public
  • protect against fraud and legal liability

The security of your data is important to us but remember that no method of transmission over the Internet, or method of electronic storage is 100% secure. While we strive to use commercially acceptable means to protect your Personal Data, we cannot guarantee its absolute security.

Service Providers

We may employ third-party companies and individuals to facilitate or improve our Services (“Service Providers”), to provide the Services on our behalf, to perform Service-related services or to assist us in analyzing how our Services are used.

These Service Providers have access to your Personal Data only to perform these tasks on our behalf and are obligated not to disclose or use it for any other purpose.

We don’t sell any of your information to anyone, and we never will.

Our Products may contain links to other sites that are not operated by us. If you click on a third-party link, you will be directed to that third party’s site. We strongly advise you to review the Privacy Policy of every site you visit.

We have no control over and assume no responsibility for the content, privacy policies or practices of any third-party sites or services.

Children’s Privacy

Our Service does not address anyone under the age of 18 (“Children”).

We do not knowingly collect personally identifiable information from anyone under the age of 18. If you are a parent or guardian and you are aware that your Children have provided us with Personal Data, please contact us. If we become aware that we have collected Personal Data from children without verification of parental consent, we take steps to remove that information from our servers.

Multiple Device Privacy

Our Services are available on multiple devices. The Account Owner has access and control over the Axios account and the devices on which the Services are in use. The Account Owner should maintain control over the devices that are used to access the Services and not reveal the password or details associated with the account to anyone. Axios cannot be held responsible for actions undertaken on an account under the Account Owner’s supervision.

DATA RETENTION, ACCOUNT DEACTIVATION AND DELETION

We store data securely until it is no longer necessary to provide our Service or Product, or until your account and patients files are deleted – whichever comes first. This is a case-by-case determination that depends on things like the nature of the data, why it is collected and processed, and relevant legal or operational retention needs.

CHANGES TO THIS PRIVACY POLICY

We may update our Privacy Policy from time to time. We will notify you of any changes by posting the new Privacy Policy on this page.

We will let you know via email and/or a prominent notice on our Services, prior to the change becoming effective and update the “effective date” at the top of this Privacy Policy.

You are advised to review this Privacy Policy periodically for any changes. Changes to this Privacy Policy are effective when they are posted on this page.
